A completed Grant Evaluation
Report is required for a past grant before a new grant
proposal can be considered. This report is due within one year
of the grant’s award date.

expenditures of all grant funds to date.
2. List the major objectives of this grant and
the ways that they were achieved. What factors contributed to

3. List other funding sources received for the
project/program. If this project is expected to be an ongoing
effort, how do you plan to fund it in the future?
4. What are some of the success stories for
this year’s activities? Are you working with any other
agencies to help you meet your goals?
5. If you have any pictures that highlight
recent events or activities, please submit them to us.
